This type of plan allows the most flexibility in choosing a dentist. A typical benefit design may look like this:

Each insurance company will offer different variables to the above design. Some have a $1,500 annual maximum available. Others will waive the deductible on the preventive services. They may offer an Orthodontia benefit.

dot Prepaid HMO Dental Plans


This type of structure works similar to an HMO medical plan and is the most economical type of dental plan. This should result in the lowest out of pocket cost to the member.


Members will choose a dentist from a list of participating dentists, then use this dentist when seeking dental care. These dentists have agreed to provide dental care at substantially reduced rates. The member will be required to pay the co-pay shown on the co-payment schedule for covered services.
